Yes, I am an MD/PhD, did a post-doc. Primary care or general surgery are not the right specialties for MD/PhDs. These departments at most academic institutions do not value basic science research much, and want a LOT of clinical time from you (50+ hours, not 1-2). There is no specialty where you can...

I agree with you about a bleak PhD job outlook. The number of PhDs far outstrips the number of available faculty positions, and PhD programs don't train people for non-academic positions. Yes, I think you would have a good shot. The most important thing you haven't mentioned is a letter of recommendation from your mentor. A couple of comments on this. First you are spending a heck a lot of money for college, then likely $100k on med school.
